2023-2024 MBA Director Forms MHSAA Band Manual 2023-2024 Director To Do List (with invoices) 1. Director Registration Form (Complete a separate form for each director. Dues must be sent to the MBA P.O. Box Make checks payable to the Mississippi Bandmasters Association) 2. MBA State Band Clinic Student Nomination List This form should be submitted BY SEPTEMBER 15. Late responses will not have a student selected. Directors must have paid their $100 MBA dues in order for their nominations to be accepted. High schools with less than 8 names listed will only have ONE student selected. Junior high/middle schools must also submit 8 names; one student will be selected. 3. MHSAA State Marching Evaluation Entry Form Due by September 15. Print confirmation and mail with $200 Entry fee to your SITE HOST. (Payable to REGIONAL SITE HOST SCHOOL) Invoice below if needed. 4. MHSAA Band Bulletin IV MHSAA State Band Evaluation Registration Complete this form and Mail fees (Invoice Below) to the MHSAA in Clinton with fees. 5. MHSAA / MBA State Marching Championship Declaration Form Due by September 15 for those interested in participating. Complete and Mail $250 to MBA. DO NOT COMBINE THIS FEE WITH ANY OTHERS. IT IS DEPOSITED INTO A SEPARATE ACCOUNT. PLEASE MAKE THIS A SEPARATE CHECK. 6. MHSAA Eligibility Instructions MHSAA Eligibility is completed through Dragonfly. Please see your administration for more guidance. 7. W9 Form PRINT AND SUBMIT THIS FORM TO YOUR BOOKKEEPER TO UPDATE THE MBA’S VENDOR ADDRESS AT YOUR SCHOOL. This form helps to ensure checks made to the Mississippi Bandmasters Association are sent to the right address. INVOICES - IF NEEDED 8. MBA Dues Invoice - Optional - If you are using a purchase order from your school, fill this out and submit this invoice to your bookkeeper if needed to authorize payment to the MBA. 9. MHSAA Band Bulletin IV Invoice - If you are using a purchase order from your school, fill this out and submit this invoice to your bookkeeper if needed to authorize payment to THE MHSAA. 10. MHSAA State Marching Evaluation Invoice - Optional - If you are using a purchase order from your school, fill this out and submit this invoice to your bookkeeper if needed to authorize payment to your regional site. 11. MBA State Marching Championship Invoice - Optional - If you are using a purchase order from your school, fill this out and submit this invoice to your bookkepper if needed to authorize payment to the MBA. 12. MBA State Band Clinic Student Registration Fee Invoice - Optional - If you are using a purchase order from your school to pay for student clinic fees, fill this out and submit to your bookkeeper if needed to authorize payment for Student Clinic Registration.
